pkg/api/api0: Refactor server list updates
* Merge update and replace into ServerHybridUpdatePut.
* Integrate limits into ServerList itself.
* Allow new servers to replace live servers if gameserver and
authserver IP and port are identical.
* Move server token generation into ServerList.
